HMS Vanguard and Le Triomphant submarine collision - Wikipedia
Two subs collided in 2009, basically because they were so good at their jobs (plus France and the UK weren’t sharing information on their submarine patrols).
There’s only one other recorded sub-on-sub collision since 2000, and that one was largely blamed on the crew of one of the two U.S. subs involved.
